You have two options:

Bring in the form along with two forms of identification to 1805 North Carson Street, Carson City, Nevada during our business hours:

- current driver’s license or state identification card, armed forces, government, university, or recognized corporation identification card, passport, alien registration card or certificate of naturalization, current lease, mortgage or Deed of Trust, voter or vehicle registration card, home or vehicle insurance registration card.

Fill-out USPS Form 1583 and sign it before a Notary Public.
Fill out our Mailbox Agreement.
Fill out Mail Forwarding and Non USPS Package Receiving Agreements if needed.
Mail your notarized USPS Form 1583, Box Agreements and payment to:
